What is Shinrai Facilities?

What is ICF-DDH / DDN

1. ICF (Intermediate Care Facility)

  •    A 24-hour residential care facility

  •    Nurses and medical staff are on-site or on-call at all times

  •    Provides medication management, health monitoring, and regular medical support

  •    Emergency protocols are established, with coordination to physicians and hospitals when needed

2. DDH (Developmental Disabilities Habilitative)

  •    A residential facility for individuals who require special developmental support

  •    In addition to meals and daily living assistance, health monitoring and medical care are provided as needed

  •    Maintains a small, family-like living environment while balancing both medical and daily living support

3. DDN (Developmental Disabilities Nursing)

  •    A residential facility with a strong emphasis on medical care

  •    Supports individuals who require daily medical care such as ventilators, suctioning, or tube feeding

  •    Nurses provide specialized care integrated with daily living support under physician supervision

How It Differs from General Care Homes

 •    General care homes focus mainly on daily living support

What is ADHC (Adult Day Health Care)

ADHC (Adult Day Health Care) is a type of care facility that individuals attend during the day. While continuing to live at home, participants come to the center during daytime hours to receive comprehensive support—medical care, rehabilitation, daily living assistance, and social engagement.

Key Features

  •    Medical Support

ADHC offers a medical model of adult day services.

Nurses are on-site throughout the day to perform vital sign checks, manage medications, and closely monitor health conditions.

Physicians and therapists provide regular assessments, and necessary treatments are administered under physician supervision.

When urgent care is required, ADHCs coordinate directly with hospitals and healthcare providers, ensuring immediate response.

  •    Rehabilitation

Licensed physical and occupational therapists provide customized rehabilitation programs to help maintain, restore, or improve mobility, strength, and independence.

  •    Daily Living Support

Assistance with meals, bathing, hygiene, and exercise is provided in a safe and supportive environment.

  •    Chronic Condition Management

ADHCs specialize in caring for individuals with chronic illnesses (e.g., diabetes, heart disease, respiratory conditions), providing continuous monitoring and preventive interventions.

  •    Social Interaction

Recreational and group activities stimulate the mind, encourage friendships, and prevent social isolation.

  •    Transportation Services

Door-to-door transportation is often provided, reducing the burden on families and making access to care easier.

Services of ICF-DDH and ICF-DDN

 

 ICF-DDH (Developmentally Disabled – Habilitative)

 

Purpose: To provide a living environment for individuals with developmental disabilities while offering habilitation—programs that help them acquire and maintain skills for greater independence.

 

Services (including medical aspects):

    •    Provision of nutritionally balanced meals and feeding assistance

    •    Daily living support: bathing, dressing, toileting, hygiene, and mobility

    •    Health monitoring, vital checks, and medication management by nursing staff

    •    On-call medical interventions when necessary

    •    Habilitation services such as physical therapy, occupational therapy, and speech therapy

    •    Recreational and community activities that encourage social participation

    •    Family support including counseling, respite, and caregiving guidance

 

Unlike a general group home, which focuses mainly on daily living support and relies on outside medical providers, ICF-DDH integrates daily living support, medical management, and habilitation within the same facility.

 ICF-DDN (Developmentally Disabled – Nursing)

 

Purpose: To serve individuals who require continuous, advanced medical care on a daily basis, by combining residential living with full nursing support.

 

Services (medical-centered):

    •    24/7 nursing presence, with continuous health monitoring

    •    Medication management, injections, IVs, blood sugar checks, seizure and respiratory care

    •    Daily medical procedures such as ventilator management, tracheostomy care, suctioning, and tube feeding

    •    Coordination of physician visits, rehabilitation therapies, and hospital care

    •    Daily living support (bathing, toileting, mobility, dressing) integrated with medical care

    •    Emergency response with direct coordination with hospitals and physicians

    •    Family training and guidance in medical and caregiving techniques

Unlike a general group home, which cannot provide complex medical care, ICF-DDN offers advanced, ongoing nursing and medical services as part of daily living support.

 Key Difference
  • Group Home → Primarily daily living support, external medical care required.

  • CF-DDH → Daily living support + habilitation programs + basic medical oversight.

  • ICF-DDN → Daily living support + continuous nursing and advanced medical care
